To share health summaries with your healthcare providers or caregivers, you can download summary reports or an active medication list in Primary Record. The health summary report includes the most important information for healthcare interactions using your data.
Download a health summary or medications list
- On a mobile device or computer, open a browser, and sign into Primary Record.
- Click Profiles >Choose the Medical Profile where you want to create the report.
- On the left side, click Reports.
- Choose one of the following:
- Summary report: Creates a list of active medications, allergies, conditions, and visits with the provider and notes.
- Medications list: Creates a list of active medications with the dosage instructions, prescriber, source, dates started and last filled.
- Click Create.
- When the report’s ready, click View report.
Create a QR code
To share your report with someone else electronically, you can create a link or QR code.
- On a mobile device or computer, open a browser, and sign into Primary Record.
- Click Profiles >Choose the Medical Profile where you want to create the report.
- On the left side, click Reports.
- Choose one of the following:
- Summary report: Creates a list of active medications, allergies, conditions, and visits with the provider and notes.
- Medications list: Creates a list of active medications with the dosage instructions, prescriber, source, dates started and last filled.
- Click Create link/QR code.
- Agree to the consent, and click Create.
