Create a Primary Record account

Use Primary Record as a central hub for electronically collecting, organizing, and sharing health information with your family, caregivers, and other care team members. You can seamlessly carry your family’s or your electronic medical narrative wherever you go, making sure you have a comprehensive and accessible healthcare record.

To easily collect the electronic health data, you can search for and connect to patient portals. You can also add context and edit the health information through notes, and invite trusted collaborators to help coordinate care.

Sign in to Primary Record

Important: Only people who are 13 years old or older can sign up for a Primary Record account.

  1. On a mobile device or computer, open a browser, and go to www.primaryrecord.app
  2. You can sign up for an account 2 different ways:
    •  Use a preferred email, and choose a unique password.
    •  Choose a Google or Apple sign-in option.
  3. Verify your account through a security code in your email, by entering an Apple passcode, or by signing in to your Google Account.
  4. Once you’re signed in, add your full name and date of birth.
  5. You finished signing in to your Primary Record account.

When you sign up for a Primary Record account, you receive one free Medical Profile for yourself or someone else. To add more Medical Profiles, you can subscribe to a Family Plan. If you pause or cancel the subscription plan, the Medical Profiles you created and all the medical information are still in your Primary Record to manually manage.

Edit your account information

To update your basic information, you can edit your Primary Record account. 

Important: To get accurate medical information from your patient portal, your legal name, email, and date of birth are required.

  1. On a mobile device or computer, open a browser, and sign into Primary Record
  2. Click Account > Account Information > Edit
  3. To edit your information, click the section you want to edit.
    • Account photo: To delete the current photo, click X, and upload a new one.
    • Email 
    • First or last name
    • Preferred name
    • Phone number
    • Date of birth
    • Address
  4. After you’re done with your edits, click Save.
